Alignment, Collaboration, and Synergy

Coordinated efforts to identify and meet the social needs of patients and population groups can lead to improved health status and well-being as well as lower health care utilization and costs. Research in this S4A priority area evaluates the combinations of health care, social services, and public health services that yield desired outcomes for specific population groups, identifies effective organizational models and financing strategies that support coordinated medical, public health and social services delivery, and how optimal models and strategies vary based on community resources or other contextual factors.
