Classifying and Comparing Hospital-to-Community Care Transition Strategies in Real-World Settings: Lessons from Project Achieve (NHPC 2018)

Overview

Project ACHIEVE examines the comparative effectiveness of care transition programs designed to help hospitalized patients and their caregivers navigate successfully back to home and community settings. Supported by the Patient Centered Outcomes Research Institute (PCORI), ACHIEVE compares the strategies implemented in care settings across the U.S., giving special attention to the needs, preferences, and experiences of patients and caregivers. In this session we share results from ACHIEVE's retrospective analysis that classifies and compares care transition strategies in use in more than 400 hospitals and care settings across the U.S. Using five years of Medicare claims data, we examine differences in the design and content of these strategies, and estimate their impact on unplanned hospital readmissions.
