Mixed methods results from a four-state, multi-PBRN study of primary care-public health collaboration at the local level will be presented. Discussion will feature an emerging, multi-dimensional partnership model, classification of local jurisdictions within that model, characteristics of successful jurisdictions, and potential barriers and solutions.
