Systems for Action released nine calls for proposal (CFPs) over the course of the program. While some requirements changed over the years, every CFP required applicants to propose a novel mechanism to align the medical, public health, and social services systems.

S4A funded a mix of one-year Pilot Studies which aimed to evaluate the acceptability and feasibility of their proposed systems alignment intervention and Impact Studies which lasted between three and five years and evaluated the outcomes of the systems alignment intervention. In 2018, S4A funded Collaborating Research Centers to conduct systems alignment research, as well as support other S4A grantees by offering mentoring on access to data resources, research design, and communication and dissemination mechanisms.
